Lactate is a naturally occurring metabolic intermediate being investigated for its potential to mitigate endothelial ischemia-reperfusion (I/R) injury. In clinical research led by Gary Van Guilder, lactate is administered intravenously to reach systemic concentrations of approximately 3 mmol/L. The therapeutic rationale is based on lactate's role as a preferred oxidative fuel for tissues under metabolic stress and its potential signaling properties that may protect the vascular endothelium from damage caused by restricted blood flow and subsequent reperfusion. The study evaluates lactate both as a standalone treatment and in combination with ischemic preconditioning (IPC), particularly in populations with high cardiovascular risk such as those with metabolic syndrome.
