Lactated Ringer's solution (LRS) is a sterile, isotonic crystalloid intravenous fluid composed of sodium chloride, sodium lactate, potassium chloride, and calcium chloride in water. It is a standard therapy for fluid resuscitation and electrolyte replenishment. In a clinical trial led by Changhai Hospital (NCT05772234), LRS is being investigated as an aggressive intravenous hydration strategy to prevent acute pancreatitis following extracorporeal shock wave lithotripsy (ESWL) for pancreatic stones. The treatment protocol involves a high-volume bolus (20 mL/kg) followed by a maintenance infusion (3 mL/kg/h), administered in combination with rectal indometacin. LRS is hypothesized to be superior to normal saline in this context because its lactate component is metabolized into bicarbonate, providing a buffering effect that may mitigate systemic inflammation and improve pancreatic microperfusion.
