Overview

Lactitol is a synthetic disaccharide analogue of lactose, chemically classified as a sugar alcohol (polyol). It functions primarily as an osmotic laxative and a prebiotic. Upon oral administration, lactitol remains unabsorbed in the upper gastrointestinal tract and reaches the colon, where it is fermented by saccharolytic bacteria such as *Bifidobacterium* and *Lactobacillus* into short-chain fatty acids. This fermentation process acidifies the colonic contents, which facilitates the conversion of ammonia (NH3) into non-absorbable ammonium ions (NH4+), effectively trapping it in the gut for excretion. This mechanism is central to its use in treating hepatic encephalopathy. Additionally, the osmotic effect of lactitol and its fermentation products increases intraluminal pressure and water content, stimulating peristalsis to relieve constipation. Emerging research also indicates that lactitol may improve the prognosis of hepatocellular carcinoma by modulating the gut microbiota, specifically increasing the abundance of *Megasphaera* and *Bifidobacterium*.
