Overview

**lactobacillus acidophilus** is a probiotic bacterial drug substance consisting of live *Lactobacillus acidophilus*, a lactic-acid-producing commensal microorganism used to modulate the gut or vaginal microbiome and downstream host immune responses. Its therapeutic rationale is not based on a single human molecular target but on microbiome-mediated effects such as competitive exclusion of pathogens, reinforcement of epithelial barrier integrity, production of antimicrobial and metabolic products, and attenuation of pro-inflammatory signaling including IL-23/Th17, TNF-alpha, NF-kappaB, and STAT3-associated pathways in preclinical and clinical studies. Publicly described uses include gastrointestinal disorders such as ulcerative colitis, irritable bowel syndrome with diarrhea, *Helicobacter pylori* eradication support, and other exploratory settings including polycystic ovary syndrome, postoperative delirium, nonalcoholic steatohepatitis, and rectal cancer adjunctive therapy. A named marketed product association is Ardeypharm's Paidoflor, and the organism is also used in vaginal formulations such as Gynoflor together with estriol, although the exact queried drug here is the single active organism rather than the combination product.
