Lactobacillus fermentum postbiotic (specifically derived from *Limosilactobacillus fermentum*) is a preparation of inanimate microorganisms and/or their components that confers a health benefit on the host. It is typically prepared as a cell-free supernatant containing bioactive metabolites such as functional proteins, extracellular polysaccharides, enzymes, cell wall fragments, and short-chain fatty acids. In preclinical research, particularly in models of cisplatin-induced chronic kidney disease (CKD), this postbiotic has demonstrated potent renoprotective, antioxidant, and anti-inflammatory properties. Its mechanism involves the reduction of blood urea nitrogen (BUN) and creatinine levels, suppression of reactive oxygen species (ROS) formation, and inhibition of lipid peroxidation. Furthermore, it enhances the host's antioxidant defense system by increasing glutathione (GSH) levels and total antioxidant capacity, likely through the activation of the Keap1/Nrf2/ARE signaling pathway.
