Lactobacillus gasseri RW2014 (LGA) is a specific probiotic strain isolated from the feces of healthy breastfeeding infants, currently under investigation for its potential therapeutic effects in metabolic diseases. LGA demonstrates significant acid and bile salt tolerance and possesses endogenous bile salt hydrolase (BSH) activity. Its primary mechanism of action involves the BSH-mediated hydrolysis of conjugated bile acids into free bile acids, which are less efficiently reabsorbed in the ileum and are subsequently excreted in the feces. This increased excretion forces the liver to utilize systemic cholesterol to synthesize new bile acids, thereby lowering serum total cholesterol, triglycerides, and LDL-C. Additionally, LGA administration has been shown to modulate the gut microbiota composition—specifically increasing the abundance of beneficial genera such as *Bacteroides*, *Parabacteroides*, and *Blautia*—and to elevate the production of anti-inflammatory short-chain fatty acids (SCFAs) like butyrate, which helps alleviate hepatic steatosis and systemic inflammation.
