Overview

Lactobacillus kefiranofaciens ZW18 is a probiotic strain whose postbiotic derivatives (referred to as Post18) are being investigated for their therapeutic potential in treating hepatocellular carcinoma (HCC). The postbiotic contains bioactive compounds such as cyclic di-GMP (c-di-GMP) and 3-hydroxybutyric acid, which possess anticancer properties. Preclinical studies demonstrate that Post18 effectively inhibits HCC cell proliferation and migration while inducing apoptosis. Its mechanism of action involves modulating the tumor microenvironment by enhancing the infiltration of CD4+ and CD8+ T lymphocytes and elevating the production of pro-inflammatory cytokines like interferon gamma (IFN-γ) and tumor necrosis factor alpha (TNF-α). Additionally, it reduces the presence of immunosuppressive regulatory T cells (Tregs) and promotes a beneficial gut microbiota profile that increases the production of short-chain fatty acids (SCFAs), particularly butyric acid, which further contributes to its antitumor effects.
