Overview

Lactobacillus plantarum (LBP) is an investigational live biotherapeutic product consisting of two genetically similar probiotic strains: Lp299 (DSM 6595) and Lp299v (DSM 9843). Developed by the Children's Oncology Group (COG), this intervention was designed to prevent acute gastrointestinal graft-versus-host disease (aGvHD) in pediatric and young adult patients undergoing allogeneic hematopoietic cell transplantation. The biological mechanism of LBP involves the preservation of intestinal microbiota diversity and the maintenance of gut barrier integrity, which are often compromised during conditioning therapy and transplant, leading to inflammatory cascades and GvHD. A Phase 3 randomized, placebo-controlled trial (ACCL1633) evaluated the efficacy of LBP administered from the start of conditioning through day 56 post-transplant. However, results published in 2025 indicated that LBP did not significantly reduce the incidence of GI aGvHD compared to placebo, although it maintained a safety profile similar to the control group.
