Overview

Lactobacillus supernatant is a cell-free preparation containing the secreted metabolites, proteins, and signaling molecules produced by *Lactobacillus* species during fermentation. In research targeting systemic lupus erythematosus (SLE), supernatants derived from a consortium of five strains—*Lactobacillus reuteri, Lactobacillus oris, Lactobacillus johnsonii, Lactobacillus gasseri,* and *Lactobacillus rhamnosus*—have been evaluated for their ability to modulate the immune system. While live bacterial mixtures of these strains have demonstrated therapeutic potential by increasing regulatory T cells and restoring the balance of T helper 17 cells in lupus-prone mouse models, the supernatant alone failed to significantly attenuate disease symptoms in some studies. This suggests that the therapeutic efficacy of *Lactobacillus*-based microbiome therapies may depend on live bacterial interactions, structural components of the cell wall, or specific community-driven metabolic processes rather than secreted factors alone.
