Laroprovstat + rosuvastatin is a fixed-dose combination (FDC) therapy being developed by AstraZeneca for the treatment of hypercholesterolemia and the reduction of cardiovascular risk. The combination consists of laroprovstat (also known as AZD0780), an oral small molecule inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9), and rosuvastatin, a potent HMG-CoA reductase inhibitor (statin). Laroprovstat works by blocking PCSK9, which otherwise mediates the degradation of low-density lipoprotein receptors (LDLR), thereby increasing the clearance of LDL cholesterol from the bloodstream. Rosuvastatin complements this mechanism by inhibiting the rate-limiting enzyme in hepatic cholesterol synthesis. Clinical data has demonstrated that the addition of laroprovstat to a rosuvastatin regimen can achieve a total LDL-C reduction of approximately 80%. The combination is currently being evaluated in Phase 3 clinical trials for several indications, including heterozygous familial hypercholesterolemia, acute coronary syndrome, and atherosclerosis.
