Overview

LB2102 is an investigational autologous chimeric antigen receptor T-cell (CAR-T) therapy targeting delta-like ligand 3 (DLL3), a Notch pathway ligand overexpressed in small-cell lung cancer and other neuroendocrine malignancies while largely absent from normal adult tissues.[1][12][14] Patient T cells are collected by apheresis and genetically engineered ex vivo to express a DLL3-specific CAR and are further “armored” with a dominant-negative transforming growth factor beta receptor 2 (dnTGFβR2) to resist immunosuppressive TGF-β signaling in the tumor microenvironment, then expanded and reinfused following lymphodepleting chemotherapy.[2][6][9] In an ongoing multicenter phase 1 trial (NCT05680922), LB2102 is being developed by Legend Biotech (in partnership with Novartis) for relapsed or refractory extensive-stage small-cell lung cancer and large cell neuroendocrine carcinoma of the lung, where it has shown early antitumor activity with partial and complete responses and a manageable safety profile characterized mainly by cytopenias and low-grade cytokine release syndrome without dose-limiting neurotoxicity.[1][2][3][4][6][8][11][12][13]
