LBL-024 + etoposide + carboplatin is a three-drug combination regimen under clinical investigation as a first-line treatment for advanced extrapulmonary neuroendocrine carcinoma (EP-NEC) and other cancers. - **LBL-024** is a novel bispecific monoclonal antibody targeting PD-L1 (Programmed death-ligand 1) and providing tumor-localized 4-1BB (CD137) co-stimulation. This dual action both blocks immunosuppressive signaling by tumor cells (PD-L1 blockade) and locally activates T cells via 4-1BB, enhancing anti-tumor immune responses. - **Etoposide** is a cytotoxic agent that inhibits topoisomerase II, inducing DNA breaks and apoptosis in rapidly dividing cells. - **Carboplatin** is a platinum-based chemotherapy causing DNA crosslinking and cell death in tumor cells. The triplet is administered intravenously and has demonstrated a promising safety profile and high tumor response rates (ORR up to 83.3% and DCR 100% in EP-NEC), outperforming historical chemotherapy outcomes. The combination is being developed and trialed primarily by Leads Biolabs and is currently in phase Ib/II and pivotal single-arm trial stages, with orphan drug and breakthrough therapy designation for neuroendocrine carcinoma in China and the US[4][6][7][9].
