LCL161 + gemcitabine + nab-paclitaxel is an investigational combination therapy consisting of three agents—LCL161, gemcitabine, and nab-paclitaxel—studied for advanced solid tumors including metastatic pancreatic cancer and drug-resistant cholangiocarcinoma. LCL161 is a small molecule SMAC (Second Mitochondria-derived Activator of Caspases) mimetic that antagonizes inhibitor of apoptosis proteins (cIAP1, cIAP2, XIAP), resulting in apoptosis induction and modulation of NF-κB signaling[1][7][9]. Gemcitabine is a nucleoside analog that is incorporated into DNA and inhibits DNA synthesis, leading to cell death[2]. Nab-paclitaxel is paclitaxel bound to albumin nanoparticles, facilitating targeted cytotoxic activity against rapidly dividing cancer cells[2]. The combination is primarily being developed to overcome drug resistance in solid tumors by targeting resistance mechanisms (ABCB1/MDR1, BIRC5/Survivin), improving sensitivity to chemotherapy, and enhancing tumor cell apoptosis[1][4].
