Overview

LDEPA (Low-Dose Eicosapentaenoic Acid) is a specific oral nutritional formulation consisting of capsules containing 0.125g of eicosapentaenoic acid (EPA) and 0.125g of olive oil. Developed and evaluated by Sun Yat-sen University, it is used as an immunomodulatory and nutritional intervention for patients with gastric or colorectal cancer undergoing XELOX chemotherapy (oxaliplatin and capecitabine). EPA is an omega-3 polyunsaturated fatty acid (PUFA) that modulates the immune system and antagonizes tumor-induced inflammatory and metabolic changes, such as cachexia. It acts by competing with arachidonic acid for metabolic enzymes, thereby reducing the production of pro-inflammatory eicosanoids and promoting the synthesis of anti-inflammatory mediators. The LDEPA regimen typically involves the administration of 24 capsules per day for 21 days to improve nutritional status, immune function, and quality of life while potentially mitigating chemotherapy-related side effects.
