Ledelabricin alfa (recombinant human lubricin, rhPRG4) is a recombinant form of the endogenous human glycoprotein proteoglycan 4 (PRG4), also known as lubricin. It is a mucin-like glycoprotein that acts as a natural boundary lubricant and anti-adhesive agent on biological surfaces, particularly in joints and the ocular surface. Developed by Lubris BioPharma in collaboration with Dompe Farmaceutici, ledelabricin alfa is designed to restore lubrication, reduce friction, and exert anti-inflammatory effects. It has been investigated for ophthalmic indications such as dry eye disease and ocular graft-versus-host disease (oGVHD), as well as for osteoarthritis, xerostomia, and surgical adhesions. Although a specific formulation (ECF843) was licensed and subsequently discontinued by Novartis for dry eye disease after failing to meet endpoints in a Phase IIb trial, development of ledelabricin alfa continues in Phase II trials for ocular graft-versus-host disease and Sjögren's related dry eye disease.
