Lenacapavir is a first-in-class, long-acting HIV capsid inhibitor developed for the treatment of human immunodeficiency virus type 1 (HIV-1) infection. It works by binding directly to the interface between HIV-1 viral capsid protein (p24) subunits in hexamers, interfering with multiple essential steps of the viral replication cycle—including nuclear import of proviral DNA, virus assembly and release, production of capsid protein subunits, and proper capsid core formation. Lenacapavir demonstrates potent antiviral activity even against multidrug-resistant strains and has an extended pharmacokinetic profile allowing for infrequent dosing (oral or subcutaneous). It is indicated in combination with other antiretrovirals for adults with multi-drug resistant HIV infection where current therapy is ineffective due to resistance or intolerance.
