Overview

Lenalidomide + G-CHOP is a combination regimen used in the treatment of certain lymphomas, particularly newly diagnosed diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma. The regimen consists of lenalidomide, an immunomodulatory small molecule with anti-neoplastic and anti-angiogenic properties, combined with the G-CHOP chemotherapy protocol. G-CHOP stands for obinutuzumab (a type II anti-CD20 monoclonal antibody), cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline antibiotic), vincristine (a mitotic inhibitor), and prednisone (a corticosteroid). Obinutuzumab replaces rituximab in this variant of CHOP regimens. Lenalidomide modulates the immune system by enhancing T cell and natural killer cell function, inhibiting angiogenesis, and directly inducing apoptosis in malignant cells. Obinutuzumab targets CD20 on B-cells to induce direct cell death and enhance antibody-dependent cellular cytotoxicity. The other agents act through cytotoxic mechanisms to kill rapidly dividing cancer cells[1][9][10]. This combination aims to improve response rates over standard regimens.
