Overview

Lenalidomide + plerixafor + rituximab is a combination regimen comprising three distinct agents with complementary mechanisms of action. Lenalidomide is an immunomodulatory small molecule that enhances T cell and natural killer (NK) cell function and modulates cytokine production by binding to cereblon and altering the activity of the cullin ring E3 ubiquitin ligase complex. This leads to degradation of key transcription factors involved in B-cell survival[2][6]. Rituximab is a monoclonal antibody targeting CD20 on B lymphocytes; it induces cell death through direct cytotoxicity and antibody-dependent cellular cytotoxicity (ADCC)[1][3]. Plerixafor is a small molecule antagonist of C-X-C chemokine receptor type 4 used primarily for hematopoietic stem cell mobilization. The combination has been studied in various hematologic malignancies such as follicular lymphoma and mantle cell lymphoma for its synergistic effects on immune activation and tumor cell killing[1][2][3].
