lenalidomide + R-CHOP

lenalidomide + R-CHOP (often referred to as LR-CHOP21 in clinical trials) is an investigational chemoimmunotherapy regimen that combines the standard-of-care R-CHOP backbone (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) with the immunomodulatory drug lenalidomide. This combination is primarily developed for the treatment of elderly patients with previously untreated aggressive B-cell lymphomas, including diffuse large B-cell lymphoma (DLBCL) and high-grade follicular lymphoma. The rationale for adding lenalidomide to R-CHOP lies in its pleiotropic anti-tumor effects, which include direct inhibition of malignant B-cell proliferation, anti-angiogenic activity, and enhancement of the immune microenvironment by stimulating NK cell and T-cell mediated cytotoxicity. The regimen is typically administered in 21-day cycles, with lenalidomide given orally and the R-CHOP components administered via intravenous and oral routes.
