Lenalidomide + rituximab + cyclophosphamide is a combination regimen used primarily in the treatment of indolent B-cell non-Hodgkin lymphoma (NHL), including follicular lymphoma, marginal zone lymphoma, Waldenström's macroglobulinemia, and other low-grade B-cell lymphomas. Lenalidomide is an immunomodulatory agent that enhances immune cell function and has direct anti-tumor effects. Rituximab is a monoclonal antibody targeting CD20 on B cells, leading to their destruction via immune-mediated mechanisms. Cyclophosphamide is an alkylating agent that interferes with DNA replication and induces apoptosis in rapidly dividing cells. The combination leverages complementary mechanisms—immune modulation (lenalidomide), targeted B-cell depletion (rituximab), and cytotoxic chemotherapy (cyclophosphamide)—to improve response rates in patients with previously untreated or relapsed/refractory indolent NHL[1][5]. Clinical studies have shown high overall response rates and manageable toxicity profiles for this regimen.
