Overview

Lenalidomide + CHOEP is an investigational chemoimmunotherapy regimen evaluated as a first-line treatment for patients with stage II-IV peripheral T-cell lymphoma (PTCL). This combination integrates lenalidomide (Revlimid), an immunomodulatory small molecule, with the standard CHOEP chemotherapy backbone consisting of cyclophosphamide, doxorubicin, vincristine, etoposide, and prednisone. Lenalidomide functions as a molecular glue that binds to the cereblon E3 ubiquitin ligase complex, leading to the ubiquitination and subsequent degradation of transcription factors such as Ikaros (IKZF1) and Aiolos (IKZF3), which are critical for the survival of certain lymphoid malignancies. The CHOEP components provide broad antineoplastic activity through DNA alkylation, topoisomerase II inhibition, and microtubule disruption. A Phase I/II multicenter trial led by the University of Nebraska Medical Center demonstrated clinical activity in PTCL subtypes, including PTCL-NOS, angioimmunoblastic T-cell lymphoma (AITL), and ALK-negative anaplastic large cell lymphoma (ALCL), although the regimen was associated with significant hematologic toxicity.
