Lenalidomide + pioglitazone + dexamethasone + treosulfan is a metronomic quadruple combination therapy investigated by the University of Regensburg for the treatment of relapsed or refractory multiple myeloma. This biomodulatory regimen combines the immunomodulatory effects of lenalidomide, the anti-inflammatory and potential anti-tumor properties of the PPAR-gamma agonist pioglitazone, the anti-myeloma activity of the corticosteroid dexamethasone, and the cytotoxic effects of the alkylating agent treosulfan. By utilizing low, metronomic doses, the therapy aims to provide continuous pressure on the tumor and its microenvironment while minimizing toxicity. Clinical data from a Phase I/II trial demonstrated significant efficacy in heavily pretreated patients, with a disease control rate of 91.5% and a median overall survival of 39.2 months.
