Overview

**lenvatinib + anti-PD(L)1 antibody** is a combination regimen comprised of lenvatinib, a small molecule multikinase inhibitor, and an immune checkpoint inhibitor targeting either PD-1 or PD-L1. Lenvatinib blocks multiple receptor tyrosine kinases primarily including VEGF receptors and FGF receptors, producing antiangiogenic and direct antitumor activity. Anti-PD-1 or anti-PD-L1 antibodies (such as pembrolizumab, nivolumab, toripalimab for PD-1, or atezolizumab, durvalumab for PD-L1) block the PD-1/PD-L1 axis, restoring and enhancing T-cell-mediated antitumor immune response. The combination is synergistic: lenvatinib reduces tumor-associated macrophages and modulates the tumor microenvironment to promote activation of CD8+ T cells and IFN pathway, augmenting the immune checkpoint blockade efficacy[1][3][4][5][6]. Main indications are advanced hepatocellular carcinoma (HCC), biliary tract cancer (BTC), and other solid tumors, under active investigation in clinical trials.
