Overview

This combination therapy regimen consists of the multi-kinase inhibitor lenvatinib, the PD-1 inhibitor sintilimab, and the FOLFOX chemotherapy backbone (oxaliplatin, 5-fluorouracil, and leucovorin). It is primarily being investigated as a first-line treatment for patients with metastatic intrahepatic cholangiocarcinoma (ICC). Lenvatinib targets multiple receptor tyrosine kinases involved in angiogenesis and tumor progression, including VEGFR1-3, FGFR1-4, PDGFRα, KIT, and RET. Sintilimab is a monoclonal antibody that enhances the anti-tumor immune response by blocking the programmed cell death protein 1 (PD-1) receptor. The FOLFOX component provides cytotoxic activity through oxaliplatin-induced DNA damage and 5-fluorouracil-mediated inhibition of thymidylate synthase, with leucovorin acting as a biochemical modulator to enhance the efficacy of 5-fluorouracil.
