This combination therapy, known as the GPLET regimen, is a four-drug protocol being evaluated for the treatment of advanced, unresectable, or metastatic cholangiocarcinoma. It consists of lenvatinib (a multi-kinase inhibitor targeting VEGF and FGF receptors), tislelizumab (a humanized IgG4 monoclonal antibody against PD-1), and the chemotherapy doublet gemcitabine and cisplatin. The regimen aims to combine the anti-angiogenic and immunomodulatory effects of lenvatinib with the immune checkpoint blockade of tislelizumab and the cytotoxic effects of standard-of-care chemotherapy. This synergistic approach is intended to overcome the immunosuppressive tumor microenvironment and improve clinical outcomes in biliary tract cancers. The regimen is currently being evaluated in a Phase 3 randomized, double-blind clinical trial (NCT05532059) sponsored by the Second Affiliated Hospital, School of Medicine, Zhejiang University.
