Lesinurad is a selective uric acid reabsorption inhibitor used in combination with a xanthine oxidase inhibitor (such as allopurinol or febuxostat) to treat hyperuricemia associated with gout in patients who have not achieved target serum uric acid levels with xanthine oxidase inhibitor monotherapy. Lesinurad works by inhibiting the function of two renal transporters, uric acid transporter 1 (URAT1) and organic anion transporter 4 (OAT4), which are responsible for the reabsorption of uric acid from the renal tubular lumen. By blocking these transporters, lesinurad increases urinary excretion of uric acid and lowers serum uric acid concentrations. It should not be used as monotherapy due to increased risk of kidney problems[1][2][3][4][5][6][7][8].
