**COOL-COS** is an investigational controlled ovarian-stimulation regimen combining oral letrozole, oral clomiphene, and subcutaneous corifollitropin alfa for infertility treatment. Letrozole reduces estrogen synthesis through aromatase inhibition, clomiphene provides tissue-selective estrogen-receptor antagonism that promotes endogenous gonadotropin release, and corifollitropin alfa is a long-acting follicle-stimulating hormone receptor agonist that directly supports multifollicular ovarian stimulation. The University of São Paulo evaluated this low-cost protocol in a completed Phase 1 pilot study assessing oocyte retrieval.
