Overview

This combination therapy consists of the HMG-CoA reductase inhibitor simvastatin and the aromatase inhibitor letrozole. It is currently being evaluated by Emory University in an early Phase 1 "window of opportunity" clinical trial (NCT05464810) for the preoperative treatment of postmenopausal women with stage I-III hormone receptor-positive (HR+), HER2-negative breast cancer. The study investigates whether the addition of simvastatin to standard letrozole therapy can more effectively reduce tumor cell proliferation, as measured by the Ki67 biomarker, compared to letrozole alone during a 14-day preoperative period. Beyond its traditional role in cholesterol lowering, simvastatin is hypothesized to possess antiproliferative and immunomodulatory properties that may enhance immune responses in HR+ breast cancers, which are typically less responsive to immunotherapy.
