Leucocyte-platelet rich plasma (L-PRP) is an autologous biological therapy being evaluated by the University of Campinas for the treatment of knee osteoarthritis. It is a concentrated suspension of platelets and leukocytes derived from the patient's own peripheral blood. The therapeutic effect is mediated by the release of various growth factors from platelet alpha-granules, including platelet-derived growth factor (PDGF), transforming growth factor beta 1 (TGF-β1), vascular endothelial growth factor (VEGF), and epidermal growth factor (EGF). These factors promote chondrocyte proliferation, mesenchymal stem cell recruitment, and extracellular matrix synthesis. The inclusion of leukocytes in the L-PRP formulation distinguishes it from leukocyte-poor PRP (P-PRP), with the leukocytes potentially playing a role in modulating the inflammatory milieu of the osteoarthritic joint. Clinical studies at the University of Campinas are investigating its efficacy both as a monotherapy and in combination with hyaluronic acid viscosupplementation.
