LV5FU2 is a chemotherapy regimen consisting of leucovorin (folinic acid) and 5-fluorouracil (5-FU). It is widely used in the treatment of gastrointestinal cancers, particularly colorectal and pancreatic cancers. The regimen typically involves a loading dose or short infusion of leucovorin followed by 5-FU administered as both a bolus and a continuous intravenous infusion over 46 to 48 hours. 5-fluorouracil is a pyrimidine analog that inhibits thymidylate synthase, an enzyme essential for DNA replication. Leucovorin is a reduced folate that stabilizes the inhibitory complex between 5-FU and thymidylate synthase, thereby potentiating the cytotoxic activity of 5-FU. The simplified LV5FU2 (sLV5FU2) is a modified version developed by the GERCOR group to streamline administration and reduce toxicity compared to the original de Gramont regimen.
