Overview

Leukocyte Platelet-Rich Fibrin (L-PRF) is an autologous platelet concentrate derived from a patient's own blood. It's widely used in regenerative medicine, particularly in oral and maxillofacial surgeries, to enhance tissue healing and regeneration. ## What is L-PRF? L-PRF is a second-generation platelet concentrate developed by Choukroun et al. in response to legal restrictions on blood handling[2][3]. It consists of a dense fibrin network containing platelets, leukocytes, cytokines, and adhesive proteins including fibrinogen, fibronectin, vitronectin, and thrombospondin-1[2]. This creates a 3-dimensional cross-linked fibrin matrix that serves as a binding site for both platelets and growth factors[2]. ## Preparation Process The preparation of L-PRF is straightforward: 1. Blood is drawn into test tubes without anticoagulant 2. It's immediately centrifuged (typically at 2700 rpm for 12 minutes or 1300 rpm for 8 minutes depending on the protocol) 3. During centrifugation, the blood naturally coagulates 4. The result is three layers: platelet-poor plasma at the top, the PRF clot in the middle, and red blood cells at the bottom[1][5] 5. The PRF clot is removed and can be pressed into a membrane if needed[1] ## Composition and Mechanism L-PRF contains: - Up to 50% of the leukocytes and 97% of platelets from the original blood[6] - Growth factors including PDGF, TGF-β, VEGF, EGF and others[1][6] - A fibrin matrix that acts as a reservoir for sustained growth factor release[6] The polymerization of fibrin during preparation produces a unique mechanical structure that distinguishes it from other platelet concentrates like PRP[2].
