Leukocyte-poor platelet-rich plasma (LP-PRP) is an autologous biological therapy being evaluated by the University Health Network (UHN) in Toronto for the treatment of knee osteoarthritis. Prepared from the patient's own peripheral blood using specialized separation systems like the Cervos KEYPRP Platelet Separator, LP-PRP contains a high concentration of platelets in a small volume of plasma, with the majority of pro-inflammatory leukocytes removed. Upon intra-articular injection, platelets undergo degranulation, releasing a variety of bioactive proteins including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF). These factors modulate the intra-articular environment, promoting tissue repair, reducing inflammation, and potentially slowing the progression of cartilage degeneration. In the ABLE OA clinical trial (Phase II/III), LP-PRP is specifically studied as part of a combination regimen alongside micronized lipoaspirate (LAM).
