Leukocyte-rich platelet-rich plasma (LR-PRP) is an autologous biologic therapy developed by the Istituto Ortopedico Rizzoli for the treatment of knee osteoarthritis and cartilage degeneration. It is prepared through a specific double-centrifugation protocol of the patient's own venous blood, designed to concentrate platelets while retaining a significant concentration of leukocytes. The therapeutic mechanism relies on the intra-articular release of growth factors and cytokines from platelet alpha-granules, such as platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF), which promote tissue repair, chondrocyte proliferation, and modulation of the joint's inflammatory environment. The presence of leukocytes is a key feature of this formulation, hypothesized to influence the local immune response and healing process. In clinical trials conducted at the Rizzoli Institute, LR-PRP is administered via a series of three weekly intra-articular injections to improve functional outcomes and reduce pain in patients with degenerative knee conditions.
