Levacecarnine (N-acetyl L-carnitine) is an acetylated derivative of the endogenous amino acid L-carnitine that plays a critical role in cellular energy metabolism and neuroprotection. It serves as a substrate for the mitochondrial enzyme carnitine O-acetyltransferase (CRAT), facilitating the transport of acetyl groups across mitochondrial membranes to maintain the acetyl-CoA pool and support fatty acid beta-oxidation. Beyond its primary metabolic function, levacecarnine acts as an acetyl donor for the biosynthesis of the neurotransmitter acetylcholine and exerts neuroprotective, antioxidant, and analgesic effects, partly mediated through the epigenetic regulation of histone acetylation and upregulation of metabotropic glutamate receptors. Developed initially by Sigma-Tau (now Alfasigma), levacecarnine is utilized both as a pharmaceutical drug and a widely available dietary supplement. It is primarily indicated or studied for the treatment of neuropathic pain (such as diabetic peripheral neuropathy), age-related cognitive decline, dementia, and geriatric depression.
