The regimen leverages multiple mechanisms of action to terminate persistent seizure activity: levetiracetam binds to synaptic vesicle protein 2A (SV2A) to modulate neurotransmitter release; midazolam acts as a positive allosteric modulator of GABA-A receptors to enhance inhibitory neurotransmission; phenytoin provides voltage-dependent blockade of sodium channels; and valproic acid increases GABA levels while also inhibiting sodium and calcium channels. Clinical data, particularly in pediatric populations, suggests this four-drug combination can be effective in cases where standard sequential therapy has failed.
