A combination therapeutic regimen consisting of levetiracetam, midazolam, phenytoin, and valproic acid, primarily utilized in the intensive care setting for the management of refractory status epilepticus (RSE). The regimen leverages multiple mechanisms of action to terminate persistent seizure activity: levetiracetam binds to synaptic vesicle protein 2A (SV2A) to modulate neurotransmitter release; midazolam acts as a positive allosteric modulator of GABA-A receptors to enhance inhibitory neurotransmission; phenytoin provides voltage-dependent blockade of sodium channels; and valproic acid increases GABA levels while also inhibiting sodium and calcium channels. Clinical data, particularly in pediatric populations, suggests this four-drug combination can be effective in cases where standard sequential therapy has failed.
