Overview

Levisoprenaline hydrochloride is the levorotatory enantiomer of isoproterenol, a potent and non-selective beta-adrenergic agonist. It exerts its pharmacological effects by binding to and activating beta-1, beta-2, and beta-3 adrenergic receptors with minimal activity at alpha-adrenergic receptors. Activation of beta-1 receptors in the myocardium produces positive inotropic and chronotropic effects, increasing heart rate and contractility, which is therapeutic in the treatment of bradycardia and heart block. Activation of beta-2 receptors leads to smooth muscle relaxation in the bronchioles and vasculature, resulting in bronchodilation and vasodilation. Historically used for respiratory conditions like asthma, its primary modern clinical application is in emergency cardiac care for hemodynamically significant bradyarrhythmias and as a temporizing measure in heart block until pacemaker therapy can be initiated.
