Overview

Levobetaxolol is a selective beta-1 adrenergic receptor antagonist (beta-blocker) used as an ophthalmic solution to lower intraocular pressure in the treatment of open-angle glaucoma and ocular hypertension. It is the S-enantiomer of betaxolol and considered the more active component of the racemic mixture. Levobetaxolol reduces intraocular pressure primarily by decreasing aqueous humor production, likely through antagonism of beta-adrenergic receptors that modulate cyclic AMP-mediated pathways in ciliary epithelial cells. This action results in reduced risk for optic nerve damage and visual field loss associated with elevated intraocular pressure. Compared to non-selective beta-blockers, it has fewer respiratory side effects due to its cardioselectivity and may also provide neuroprotective benefits by blocking sodium and calcium influx into neurons.
