Levobupivacaine + dexamethasone is an investigational drug combination administered intrathecally for spinal anesthesia. This specific formulation, utilizing preservative-free dexamethasone sodium phosphate and levobupivacaine, was studied by General Hospital Sveti Duh in elderly patients undergoing surgical repair for proximal femoral fractures. Dexamethasone, a synthetic corticosteroid and glucocorticoid receptor agonist, is added to the local anesthetic levobupivacaine to prolong the duration of sensory and motor blocks while simultaneously reducing the systemic stress response and neuroinflammation. The primary therapeutic goals are the management of postoperative pain and the prevention of postoperative delirium and cognitive dysfunction, which are common complications in the geriatric population following hip surgery.
