Overview

Levonadifloxacin is a novel broad-spectrum antibiotic of the benzoquinolizine subclass within the fluoroquinolone class. It is the (S)-enantiomer of nadifloxacin and exhibits enhanced potency compared to its racemic mixture. Levonadifloxacin acts primarily by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, repair, and transcription. This mechanism confers potent bactericidal activity against Gram-positive bacteria—especially methicillin-resistant Staphylococcus aureus (MRSA), quinolone-resistant Staphylococcus aureus, vancomycin-intermediate and vancomycin-resistant strains—as well as activity against certain Gram-negative pathogens and resistant respiratory bacteria such as Streptococcus pneumoniae. The drug has been developed in both intravenous (levonadifloxacin) and oral prodrug (alalevonadifloxacin) formulations to treat acute bacterial skin and skin structure infections (ABSSSI), including diabetic foot infections with or without concurrent bacteremia. Levonadifloxacin demonstrates efficacy in acidic environments relevant to infection sites such as abscesses or intracellular compartments[1][2][4][5][6][7].
