Overview

Levosulpiride is the levorotatory enantiomer of sulpiride, a substituted benzamide antipsychotic and prokinetic agent. It acts primarily as a selective antagonist of dopamine D2 and D3 receptors, with preferential binding to presynaptic autoreceptors at low doses and postsynaptic antagonism at higher doses. This dual action underlies its efficacy in both psychiatric disorders—such as schizophrenia (especially negative symptoms), major depressive disorder, anxiety disorders, dysthymia, vertigo—and gastrointestinal conditions including dyspepsia, irritable bowel syndrome (IBS), gastroparesis, gastroesophageal reflux disease (GERD), nausea/vomiting, and functional dyspepsia. Levosulpiride also exhibits moderate agonist activity at serotonin 5‑HT4 receptors contributing to its gastroprokinetic effects. It is not approved in the United States or United Kingdom but is widely marketed in Europe, South Korea, Latin America, India and Pakistan[1][2][3][4][5][7].
