LGM2605 is a synthetic small molecule version of the naturally occurring flaxseed lignan secoisolariciresinol diglucoside (SDG). It exhibits potent antioxidant and anti-inflammatory properties. Mechanistically, it acts as a free radical scavenger, activates the Nrf2/antioxidant response element (ARE) pathway, and inhibits inflammasome activation and inflammatory cell influx such as neutrophils and macrophages. These actions reduce reactive oxygen species (ROS), oxidative stress, DNA damage, and pro-inflammatory cytokine release including IL-1β, IL-6, TNFα, IL-18. It has shown protective effects in models of asbestos-induced inflammation and genotoxicity in human mesothelial cells as well as radiation-induced lung injury. The drug is being developed primarily for pulmonary diseases including asthma and pulmonary fibrosis with preclinical evidence supporting its use to improve lung function parameters especially in ex vivo lung perfusion for transplantation.
