Liangge San (also known as Liang-Ge-San or Liangge decoction) is a classic traditional Chinese medicine (TCM) formula composed of several herbal ingredients, including *Forsythia suspensa*, *Rheum palmatum*, *Scutellaria baicalensis*, *Gardenia jasminoides*, *Glycyrrhiza uralensis*, *Mentha canadensis* (or *Mentha haplocalyx*), *Lophatherum gracile*, and sodium sulfate. It is traditionally used to clear heat, purge fire, and relieve toxicity. In modern clinical research, Liangge San is being evaluated for the treatment of sepsis, sepsis-associated intestinal dysfunction, and acute lung injury. Its therapeutic effects are believed to be mediated through anti-inflammatory pathways, including the inhibition of glycogen synthase kinase-3 beta (GSK-3β) expression, suppression of the NF-κB pathway, and activation of the cholinergic anti-inflammatory pathway, which collectively reduce inflammatory cell infiltration and cytokine release.
