Overview

Lidamidine is a **small molecule antidiarrheal agent** primarily acting as an **alpha-2 adrenergic receptor agonist**. It inhibits intestinal secretion, reduces intestinal transit, and relaxes intestinal smooth muscle via peripheral alpha-2 adrenoceptor stimulation[1][3][7][8][9]. Lidamidine has minimal penetration of the blood-brain barrier and thus lacks significant centrally mediated alpha-2 agonist side effects such as hypotension and sedation[1][3]. Clinical studies have demonstrated efficacy in treating both acute and chronic forms of diarrhea, as well as gastrointestinal motility disorders, and it has been explored for use in irritable bowel syndrome[1][4][5]. The safety profile is favorable, with minimal side effects reported at therapeutic doses.
