Overview

Lidocaine + bupivacaine is a combination of two amide-type local anesthetics used to provide regional anesthesia and postoperative pain relief. Lidocaine is characterized by a rapid onset of action, while bupivacaine provides a significantly longer duration of effect. Both agents work by binding to the intracellular portion of voltage-gated sodium channels, stabilizing the neuronal membrane and preventing the influx of sodium ions necessary for the initiation and conduction of nerve impulses. This specific 50:50 mixture was investigated by King Saud University in a Phase 3 clinical trial (NCT02282774) as a subtenon block adjunct to general anesthesia for patients undergoing vitreoretinal surgery, aiming to reduce postoperative pain, the oculocardiac reflex, and postoperative nausea and vomiting.
