Overview

Lidocaine + diclofenac is a combination drug regimen investigated for the treatment of acute renal colic. Lidocaine is a small-molecule local anesthetic and antiarrhythmic that acts by blocking voltage-gated sodium channels, providing visceral and central analgesia. Diclofenac is a non-steroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX) enzymes, specifically COX-1 and COX-2, to reduce prostaglandin synthesis and provide sustained pain relief. This specific combination, involving intravenous lidocaine and intramuscular diclofenac, was evaluated in a Phase 2 clinical trial (NCT05653401) sponsored by the University of Monastir in Tunisia, where it was compared against magnesium sulfate and placebo controls in an emergency department setting.
