lidocaine + diclofenac (Hospital de Viladecans)

CLIFE1 (also known as APC201) is an investigational topical gel formulation combining the local anesthetic lidocaine (2%) and the non-steroidal anti-inflammatory drug (NSAID) diclofenac (0.5%). Developed by the Hospital de Viladecans, it is specifically designed for the management of postoperative pain following benign anorectal surgery (BARS), including procedures such as hemorrhoidectomy, fistulectomy, and fissurectomy. The mechanism of action involves lidocaine's blockade of voltage-gated sodium channels to inhibit nerve conduction and provide rapid analgesia, complemented by diclofenac's inhibition of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes to reduce inflammation and provide prolonged pain relief. In the multicenter CLIFE-2010FV clinical trial, CLIFE1 demonstrated superior analgesic efficacy compared to lidocaine-only gel (CLIFE2).
