Overview

Lidocaine hydrochloride + epinephrine bitartrate is a fixed-dose combination drug used primarily for local and regional anesthesia. Lidocaine, an amide-type local anesthetic, functions by blocking voltage-gated sodium channels in neuronal membranes, thereby inhibiting the initiation and conduction of nerve impulses. Epinephrine, a sympathomimetic amine, acts as a vasoconstrictor by stimulating alpha-adrenergic receptors. This vasoconstriction reduces local blood flow at the site of administration, which serves to prolong the duration of lidocaine's anesthetic effect, decrease the rate of systemic absorption (thereby reducing potential toxicity), and minimize surgical bleeding. The Centre Hospitalier Universitaire de Nimes has been involved in clinical research evaluating this combination for surgical anesthesia and postoperative pain management in hand and foot surgeries, often in the context of Wide Awake Local Anesthesia No Tourniquet (WALANT) techniques.
