Overview

Linagliptin (BI 1356) is an oral antidiabetic drug of the dipeptidyl peptidase-4 (DPP-4) inhibitor class. It works by inhibiting the DPP-4 enzyme to increase incretin levels (GLP-1 and GIP), which in turn increases insulin secretion and decreases glucagon secretion in a glucose-dependent manner. Glyburide is a second-generation sulfonylurea that lowers blood sugar by stimulating pancreatic beta cells to release insulin through closure of ATP-sensitive potassium channels. The combination of linagliptin and glyburide targets type 2 diabetes mellitus by improving glycemic control via complementary mechanisms—linagliptin enhances endogenous incretin action while glyburide directly stimulates insulin release from the pancreas[1][3][6]. Linagliptin was developed by Boehringer Ingelheim; glyburide is available as a generic from multiple manufacturers.
