This combination therapy regimen consists of linsitinib (OSI-906), an oral small molecule dual inhibitor of the insulin-like growth factor-1 receptor (IGF-1R) and the insulin receptor (INSR), and letrozole, a non-steroidal aromatase inhibitor. Evaluated in a Phase II trial (NCT01223690) led by the Vanderbilt-Ingram Cancer Center, the regimen was sometimes administered with the EGFR inhibitor erlotinib. The combination targets the IGF-1R and INSR pathways, which are implicated in resistance to standard endocrine therapies like letrozole in hormone receptor-positive (hormone-sensitive) metastatic breast cancer. By inhibiting these escape pathways, the treatment aims to restore or enhance the efficacy of estrogen deprivation in tumor cells.
